fix/s2-required-fields
S2 answers that omit or null a required field can attest a revision-zero item because JSON decoding fills missing integers with zero. Require explicit item_id, revision and result before matching any answer, failing malformed responses closed to UNKNOWN while preserving explicit zero revisions. Suite tests/vet and a real current Memoria consumer build/test pass; Fable review is pending and a separate CI gate prerequisite is being prepared.
