Document the flat->nested layout migration and make nested the mandated default in the operator runbooks, so the tenant-isolation defect found in the 2026-10-06 assessment (finding #1) cannot silently recur. Includes the migration plan (with the prerequisite build + guardrail spec) and the assessment report it references. Runbooks assert the mandate; code-level enforcement (plans 4-5) is a separate follow-up.
