Add platform name note to password gate login page

Show where password protection is coming from by displaying the platform name derived from CARLOSPLATFORMDOMAIN environment variable.

Examples:

• Carloku: shows "This page is password protected by Carloku"
• Tito Platform: shows "This page is password protected by Tito Platform"

Files changed:

• internal/gate/page.go: Added providerName() function and Provider field
• internal/gate/login.html: Added note showing provider when configured
• internal/gate/gate_test.go: Added tests for provider functionality
