A.4: memoria names the provider on derived artefacts, not only answers

Suite §8 says "every artefact it produced" and means the persisted
derived artefacts too, since a regenerated artefact can carry a
different provider from the one that first made it. Raised by the
memoria session.

+Decision 4 widens from *"named on every answer"* to *"named on every
+answer and every derived artefact"*, because suite §8 says "every
+artefact it produced" and means it. An answer is transient; a derived
+artefact (§5.2) outlives the query and is regenerated when a source
+changes, so it can carry a different provider from the one that first
+made it. §5.2 records the producing model alongside provenance so a
+regenerated artefact never inherits the wrong name. Raised by the memoria
