Close never waits on the log, the refusal is the door itself, no test leaks a worker

Astra on #20, round 3.

P2, close could hang on a stalled log sink: its diagnostics and the
worker's were direct logger calls. Now only a log pump writes to the
logger; the worker and close hand lines to logf, which never blocks (a
buffered channel, and a count of lines lost when it is full). close
returns within its bound and the worker exits whatever the sink does;
the pump ends with the worker.

Four test apps leaked their mail workers (three newApp calls in
auth_test.go and one in config_test.go never closed). They close in
cleanup now, and both test packages' TestMain fail the run if any
mailQueue worker or pump is still running afterwards.

P2, ?err=refused could be typed by anybody: doorOnRefusal now renders
the door in the refusal's own 403, marked through the request context;
the query no longer selects the refusal sentence.

Tests: TestMailQueueCloseIsBoundedWithAStalledLogger (close in time,
worker gone, goroutines back to baseline once the sink is released)
fails on 7ef08e7 ("close hung on a stalled log sink"); the TestMain leak
check failed with 8 queue goroutines before the cleanups;
TestKeymailRefusalIsTheDoor and the Home-mode test now require the door
in the 403 and no refusal sentence at ?err=refused, and fail on 7ef08e7.
