provider: memoria's first model client, and it fails closed

The only code in memoria that dials a model: TensorX (decision 13), one
call, §8.9's address extraction. Everything that could have been a
setting is a constant with its reason beside it — the base URL above all,
because a configurable base URL on an OpenAI-compatible client is how an
owner interposes a proxy while every page still names one provider. The
destination can be changed only through an unexported test seam.

Nothing here logs, and no returned error carries content: status codes
and fixed strings only (gate 31). Every negative test plants a canary in
the response and asserts no error contains it — which caught nothing,
because the code was written to it, but it is what will catch the next
person who wraps a decode error.

Fails closed on: a non-2xx; a body past the cap, detected by reading one
byte beyond it; a provider-marked truncation, rejected whole per §8.9
because a truncated list is exactly the silent omission §8.9 cannot
otherwise see; any finish reason but a clean stop; and a strict parse of
the model's own JSON — duplicate keys, unknown fields, trailing content,
null-for-empty — because this is the one parse in memoria whose input an
attacker may have written. §6.5's 800 ms deadline, and the caller's
context, so an abandoned keystroke cancels the call.

Four mutations run and red, each on its own case: body into the status
error, the cap undetected, truncation accepted, duplicate keys allowed.
