Move the menu bar's script onto estilo's menu markup, and rebuild the bundles

menu.mjs, shortcuts.mjs and editor-entry.js read estilo's
[estilo-menubar], [estilo-menubar-menu], [estilo-menubar-title] and
[estilo-menu-item] instead of [docs-menu*]. menu.mjs gains the check
column (aria-pressed kept in step with the view mode, the paragraph
style and the alignment, through toolbar.mjs's new isPressed), keeps a
checked item enabled ("Body text" was drawn disabled while current), and
opens the menu under the pointer while another is open. The
data-docs-unavailable path is gone with the unbuilt items. jump-to.mjs
turns File > Open a document... into the palette. Both hashed bundles are
rebuilt with fixtures/build-bundles.mjs.
