moves-phase1-6 Task 3 fix round 5 (1): move_log_outbox's NonApp reason
A moves audit outbox that stores no app name: rows bound to an instance incarnation, verified against the host's claim and record at every write, written only by the moves recovery and log passes, purged with the incarnation.
