zdd-readiness: final review round 1, the approved copy record follows the book-unreadable refusal to its shared constant
The text is unchanged. Only the record's file field moves, from internal/host/rollout.go to internal/rollout/wakeplan.go, where BookUnreadableRefusal now holds it.
