A region-placed instance is staged and routed, then never runs: five of six armed regions serve 502 Issue open

GET /api/cli/apps/<acct>/<app>/regions reports six armed regions for tito (de ie in se uk us). An instance created into one of the five non-home regions is accepted, placed, staged and routed — and then never runs. It answers 502 forever.

Reproduced live on tito today, then torn down. Everything below is observed, not inferred; where I could not establish a cause I say so.

Reproduction

carlos instances create --account tito --app titogo \
  --host region-probe.go.tito.io --region us

What worked, in order:

  1. Console accepted it. created instance region-probe.go.tito.io (channel edge).
  2. Placed on the US box. A poll of the instances API returned "region":"us", then "served_by":[{"box":"hedy-beachcombing","region":"us-east-1"}]. Placement is correct — it did not fall back to the home region.
  3. The box minted its own route. In /data/registry.db on carlos-tito-us: region-probe.go.tito.io, kind=instance, status=ready.
  4. The box staged the binary, including the root-pass self-heal. The agent runs ProtectSystem=strict with ReadWritePaths=/data /run/carlos, so /opt/carlos/versions is read-only to it. It noticed, and asked:

`` region-probe.go.tito.io: mkdir /opt/carlos/versions/a058225: read-only file system agent: region-probe.go.tito.io needs a058225 staged and this process cannot write the version store — asked for a root pass ``

carlos-stage.service (carlos adopt --stage-only) then ran as root, exited 0, and logged:

`` region-probe.go.tito.io: repointed -> a058225 (edge; hibernating — runs it on next wake) ``

The symlink is real: /opt/carlos/live/region-probe.go.tito.io -> /opt/carlos/versions/a058225/bae/titogo. This half is working exactly as designed and I want to be clear it is not the complaint.

Then it stops. Over ~15 minutes and repeated requests (which should have been wake triggers):

  • https://region-probe.go.tito.io/ → 502, every time, never anything else.
  • On the box: *no `carlos-app@ unit**, **no socket in /run/carlos/** (only the apps/ directory), **no instance database anywhere under /data** (find /data -name 'region-probe'` → nothing).

The record's health stayed Setting up and staged_version stayed empty throughout, so the console never saw it run either.

For contrast, on carlos-tito-ie the same machinery is fully alive: carlos-app@<host>.service units running, sockets in /run/carlos/, and the four platform-managed titogo instances hibernating and waking correctly.

The region's infrastructure is already there

This is not "the region was never built". On carlos-tito-us:

  • CARLOS_REGION=us
  • CARLOS_HIBERNATE_BUCKET=carlos-tito-us-665106592142, CARLOS_HIBERNATE_REGION=us-east-1, CARLOS_HIBERNATE_PREFIX=instances
  • carlos-edge.service running, ACME working, routes converging
  • the data bucket exists (arn:aws:s3:::carlos-tito-us-665106592142, us-east-1)

But that bucket is empty. IE's equivalent carries instances/, mirror-drop/ and titogo/ prefixes. Nothing has ever been written for a US-resident instance, which matches there never having been one.

So placement, staging, routing, the hibernate config and the bucket are all in place, and the one step that never happens is bringing the instance up.

I could not establish from outside whether the agent on a region box is supposed to run instances at all today, or whether it is meant to and this is a fault. That distinction is the actual question in this issue.

Why it matters beyond one probe

armed: true is, as I understand it, derived from the codes boxes publish via CARLOS_REGION into the edges fold. That makes it an edge-membership fact. But it is also what the instances API offers as "regions that can take an instance right now", and what a consumer's region picker renders to an end user.

On tito that means the API currently advertises six regions where an account can live, and five of them will accept the instance and then serve 502. From the consumer side there is no signal to distinguish them — the create succeeds, the placement is correct, the record looks healthy-ish, and the failure only shows up as a dead host.

The consumer-side work here is done and shipped (region picker, residence doors, write-freeze, the two-phase move client, the hairpin), and it is all inert until a region box can actually host. We are about to import several hundred dormant accounts and fan them out by region, so this is now the critical path rather than a future concern.

What would help

  1. Confirm the intended state: is a region box today an edge only, or is it expected to run instances? If edge-only, then armed advertising it as placement-capable is the bug, and the fix is that the regions endpoint reports hosting-readiness rather than edge membership.
  2. If it is meant to host — what is missing on carlos-tito-us such that a staged, routed, correctly-placed instance never starts? Happy to run any diagnostic on the box; I have SSM root there.
  3. One region as a pilot would unblock us — we do not need all five. us is the natural one; the probe above is a two-minute reproduction.

One sizing note, offered rather than asked: the five region boxes are t4g.small (2 GB) against IE's t4g.xlarge. Fine for an edge; on IE a titogo instance measures ~128 MB PSS, so a 2 GB box would hold a handful at most. If region boxes are to host, they will likely need resizing — but that is downstream of the question above, not part of it.

Possibly related

  • #7 (multi-region is a copy-paste exercise) — that is the Terraform shape of standing region hosts up; this is the app-hosting half, after the host exists.
  • #99 (replicated-unit route class) and #103 (edges republish on route mint/retire) — both residence-adjacent, neither covers an instance failing to start on its region box.

Filed from the tito side; happy to be told this is already known or is expected-for-now.

Answering (1) directly: a region box today is edge-only — not by design, but because nothing tells it otherwise and nothing checks. So armed advertising it as placement-capable is the bug, and that is what this fixes.

What the 502 told us

Every wake failure internal/activator's serve() has — lease held, wake timeout, spawn error, restore error — answers 503. It declines the request entirely, returning false, in exactly one case: the route is not a hibernating instance. So the 502 you saw was never a failed wake. It was the edge proxying straight to /run/carlos/<host>.sock — the path the route derives, and which only an armed activator ever creates — into the stdlib ReverseProxy's default handler. That also explains the ~13s you may have noticed per request: dialUnixWithRetry retries ENOENT for 13 seconds first.

Your empty bucket is the same finding arriving independently, and it was the most useful thing in the report. Every wake claims an S3 lease before it does anything else, so a bucket with no lock object means no wake was ever attempted on that box. Not attempted and failed — never reached.

Why

An activator arms on CARLOS_HIBERNATE_BUCKET in the box's /etc/carlos/host.env, read by carlos-edge.service at start. Nothing in the platform repo puts it there: carlos bootstrap deliberately doesn't, and infra/modules/region-host grants the host role its region's data bucket and then never mentions it again. The bring-up procedure's minimum host.env is four variables and none of them is this one. That is why the bucket exists, is granted, and is empty.

Everything else you saw follows: adoption and staging are region-blind and activator-blind, so the binary staged and the symlink repointed exactly as designed; the provisioning pass minted the route the same way; and then nothing on the box owned the process. No unit because exec-backed instances don't get one; no socket and nothing under /data because those are all minted at spawn.

What landed

Branch region-hosting-armed, CI green, not merged — it changes a production gate, so it wants your eyes.

armed now means "can host". It was "a box publishes this placement code and has an EIP" — an edge-membership fact — while the regions endpoint, the create door and every consumer picker read the set as "regions that can take an instance right now". armedPlacements now also requires the box's published data_bucket, which is CARLOS_HIBERNATE_BUCKET verbatim, written by the same process, from the same env, that decides whether the activator arms. It has been on the wire since the instance-move work; nothing read it.

It is dormant until some box publishes a data bucket, so roll order is safe, and it heals with no console change: the moment a region box's host.env grows the variable, its next edges-file pass publishes the key and that region arms itself.

The box now says it out loud. Edge Reload names a hibernate-flagged instance route on a box with no activator — which host, which socket nobody will bind, which variable fixes it. That condition was completely silent before, which is most of why this cost you fifteen minutes and an SSM session.

On (3), the pilot

It is a box-side change, not a platform one, and it is small: add CARLOS_HIBERNATE_BUCKET (and _REGION, _PREFIX) to carlos-tito-us's /etc/carlos/host.env and systemctl restart carlos-edge. The IAM grant and the bucket are already there. Watch for agent: hibernation activator armed in the journal — that line is the whole confirmation. us then re-arms itself in the console within a pass, with nothing to deploy.

Your sizing note stands and is worth acting on before real tenants: 2 GB against ~128 MB PSS per instance is a handful, and carlos-edge.service caps the whole exec-child tree at MemoryMax=80%.

Two things I found on the way, not fixed here

Per-instance DNS for a region-placed host is recorded but not built (internal/console/apiinstances.go says so in as many words). So region-probe.go.tito.io almost certainly still resolved to the home edge and arrived at the US box over the backhaul — whose dial and TLS failures are also bare 502s. If a region box turns out to be armed and still 502s, that is the next place to look, and it is the same class of problem as #103.

"Setting up" was a separate bug, and not region-specific. instanceHealth opens the console box's own local registry and joins records against it, so a route minted on any other box hits "no route minted yet" and reads Setting up forever. The fleet snapshot carries only route_versions, not the kind/status/hibernate/provisioned columns healthWord needs, so the console currently cannot compute health for any off-console-box route. Worth its own issue — say the word and I'll file it.

One correction to the framing in the report, offered gently: this was not five regions failing. It was one missing line in a hand-rolled host.env, replicated five times, that the platform had no way to notice and every reason to. The consumer-side work you shipped is not inert — it was being told the truth about edges and reading it as a truth about hosting.

Correction: my diagnosis above is wrong. Please don't act on it.

I checked the live edges files before deploying, and every tito region box publishes a data_bucket:

annie-fermenting se 16.16.113.244 carlos-tito-se-665106592142
claude-marquetry uk 35.177.77.51 carlos-tito-uk-665106592142
frances-aquascaping in 13.233.36.247 carlos-tito-in-665106592142
hedy-beachcombing us 3.92.98.101 carlos-tito-us-665106592142
linus-pottery de 63.181.123.82 carlos-tito-de-665106592142
betty-fossicking — 54.228.234.85 carlos-tito-ie-665106592142

That field is written from CARLOS_HIBERNATE_BUCKET verbatim, so the variable is set on carlos-tito-us and its activator is armed — exactly as you reported and I under-weighted. "A region box is edge-only" was wrong, and so was the host.env story I built on it. Sorry for the noise.

The change I landed (armed also requires a published data bucket) is a no-op on tito for that same reason — all six regions keep their armed flag. It is still a correct hardening, and the box-side log line it adds is still worth having, but it does not fix this issue and I have reopened the question.

What the live check does establish

DNS never points at the region box. *.go.tito.io resolves to edge.platform.tito.io → 54.228.234.85, which is betty-fossicking — the IE home box. hedy-beachcombing is 3.92.98.101. So region-probe.go.tito.io never resolved to the box that homed it. Every one of your requests landed on IE and could only have reached the US box over the backhaul.

That matches internal/console/apiinstances.go, which says so outright: per-instance DNS for a multi-region host is "Recorded, not built."

And it re-explains the 502 without needing my wrong story: a failed backhaul hop — dial or TLS verification — is a bare 502 by design, raised on the home box, with nothing written on the US box at all. That fits your empty US data bucket better than my version did, because on this reading the US box genuinely never saw a request.

Each region box currently serves exactly two routes, both everywhere-class platform names (r-n54rxsznheidq7q67h.bab.platform.tito.io, techarena.bak.platform.tito.io). No app instance has ever run on one. All 70 real hosts are on betty.

The account wildcard is mirrored (wildcard-go.tito.io.pem in control/certs/), so the obvious TLS explanation for a failing hop is probably not it — which is why I am not offering another confident answer here.

What would settle it

Your reproduction, plus three readings, which I did not have because the probe was torn down:

  1. Time one request. curl -o /dev/null -sw '%{http_code} %{time_total}\n'. ~13s means the request reached a box and died dialling a unix socket; sub-second means it died in the hop on IE.
  2. On betty (IE): journalctl -u carlos-edge | grep -E 'backhaul|proxy error' — a failed hop logs there, with the reason.
  3. On hedy (US): sqlite3 /data/registry.db "select host, kind, hibernate, socket, provisioned, region, status from routes where host='<host>'" and journalctl -u carlos-edge | grep -E 'activator|<host>' — whether the row is hibernate=1 and whether the activator ever saw a request.

If (2) shows a failed hop, this is a backhaul/DNS issue and the fix is either per-instance DNS (currently unbuilt) or whatever the hop is failing on — and it is adjacent to #103, as you guessed. If (3) shows the activator declining, the row's hibernate column is the thing to look at.

Happy to run the reproduction and these three checks myself if you'd rather — say the word. I'd rather be asked than guess twice.

The corrected version landed on main as 851a531 (CI green).

What it actually does, stated without the wrong story I told earlier:

  • Each box publishes hibernation_configured in its edges file — always, including false, because only an explicit false can distinguish "this box cannot host" from "this writer predates the field". Hosting is now answered per box and never inferred from what a different box publishes. The version I merged first got that wrong, and one box adopting a new build would have withdrawn every region that had not yet rolled.
  • The create door's bar is split by kind: every kind needs the region to be reachable, only a socket-backed instance needs it to host. A static site was being refused for want of an activator it never uses.
  • The move door requires the target's published staging bucket, unconditionally. This one was a live bug independent of anything here: the door accepted a move into a region with no data_bucket while moveTargets on the worker side drops exactly those targets, so the move sat in preparing forever with no way out but a manual cancel.
  • region-host/README.md's minimum host.env now includes the hibernation block instead of listing four variables and saying "nothing more".

None of this explains your 502, and it does not close this issue. The tito region boxes are configured correctly; that remains true. The open lead is still the one at the end of my correction: *.go.tito.io resolves to the home box and never to the region box, so a region-placed host's traffic has to cross the backhaul, and a failed hop is a bare 502 raised on the home edge with nothing written on the region box — which is consistent with the empty data bucket you found.

I could not settle it from outside: the mesh key is present, the account wildcard is mirrored, and 443 is open to the world on the region hosts, so none of the obvious hop failures fit. It needs the live reproduction plus the three readings — the request's latency, journalctl -u carlos-edge | grep -E 'backhaul|proxy error' on betty, and the registry row plus activator log on hedy. Happy to run it whenever you want, or it is a two-minute job for you since you have SSM root there.
